As a patient, you have the freedom of choice in selecting available providers. You may choose to receive treatment at any agency available, either through your insurance carrier or managed care. Treatment in this office may include a referral for further health-related services, devices, or products. Pennsylvania law requires any healthcare practitioner to disclose to you any financial interest they have in any healthcare facility in which they recommend further health-related services, devices, or products. You are not required to use the co-location, Peerstar LLC, SM, affiliated with Nulton Diagnostic and Treatment Center, PC, through common Ownership.
